SPORTS PHYSICALS
CMH is offering walk-in sports physicals for students on Aug. 12 and 13 at Astor Elementary School in Astoria. Appointments are first-come, first-served, no appointment is needed.
Date: August 12th and 13th, 2026
Location: Astor Elementary, 3550 Franklin Avenue, Astoria, OR 97103
Time: 9 AM - 12:00 PM & 1:00 PM - 4:00 PM (closed from 12:00 PM - 1:00 PM for lunch)
Cost: $30 - Contact your school if you need assistance with the cost
